FORM FOR ATHLETIC PHYSICALS


Athletic Code of Conduct

St. Thomas More Catholic High School is a member of the Louisiana High School Athletic Association and is classified as AAAAA school for athletic competition.  The school follows the rules and regulations of the LHSAA.  In addition, the school follows its own sportsmanship code that incorporates Catholic values into the athletic program.  All STM student athletes are given copies of this code, and all members of the STM family are encouraged to do everything possible to promote good sportsmanship at all school events.  Even when officiating, coaching, participation, and scheduling may not appear to conform to an individual student, teacher, or parent's preferences, STM fans and students are urged to demonstrate Christian sportsmanship, patience, and fair play. 


Eligibility

Working with each student's academic program, St. Thomas More's Department of Athletics seeks to make the STM student a well-rounded individual.  To be eligible for participation in the school's athletic program, the student must pass at least five subjects which count toward graduation and must earn at least a 1.5 grade point average in all subjects pursued each year.  Students who do not meet this minimum requirement cannot participate in st. Thomas More's athletic competition.  Athletic eligibility is based on previous semester grade point average.


ATTENTION PARENTS OF JUNIOR/SENIOR ATHLETES PROSPECTIVE COLLEGE ATHLETES NEED TO:

    • Register with the NCAA eligibility center at www.ncaaclearinghouse.net at the begninning of thir junior year. If you are a senior considering college athletics and have not registered, you need to do so immediately. This needs to be done from home because there is a concurrent fee paid at the time of registration.
    • At the same website, you need to complete the amateurism questionnaire, and or update it if you registered last year. Simply click on enter/update amateaur questionnaire.
    • At the same website, you need to print the transcript release form, sign it and bring it to your grade level guidance counselor. Transcripts are sent at the end of the junior year and final transcripts are sent after graduation.
    • Have your ACT and or SAT score sent directly to the NCAA clearinghouse. When you register for these tests there is a link for sending scores. The clearinghouse code for standarixed test scores is 9999.
    • By April 1st of your senior year, you need to log back in to the clearinghouse to update any new student information and to request a final amateurism certification.
    • Parent/Student needs to verify at the NCAA website that the core courses have been met - click on the Form 48H link to view approved STM requirements
